The Indiana Academy of Ophthalmology would like to invite you to join ophthalmologists from across the state to expand your knowledge, enhance your profession, and develop your professional network. PHYSICAL ASSISTANCE/DIETARY REQUIREMENTS - Those attendees who need additional assistance should contact the IAO Office at 317-577-3062 so that appropriate arrangements can be made.
Refund policy: All cancellations must be emailed to Kim Williams at kwilliams@thecorydongroup.com. Call in cancellations will not be accepted. Cancellations received less than 10 business days prior to seminar date will be refunded attendance fee less an $85 processing fee. However, no refunds will be made for cancellations received 5 business days or fewer prior to the seminar date. No shows and late cancellations will be mailed the seminar materials and billed the full amount of the seminar. Substitutions are allowed at any time.
SAFETY PROTOCOLS
MASK POLICY: The IAO has made the decision to follow current CDC guidelines, stating masks are recommended for attendees who are fully vaccinated or unvaccinated, and that those who are unvaccinated continue to practice social distancing. Should CDC, Indiana or local guidelines change prior to or during the conference, IAO guidelines will adjust accordingly and be communicated to attendees.
All attendees are encouraged to remain considerate of fellow attendees’ health and comfort levels. If you are feeling sick and/or believe you have been exposed to COVID-19 or any of its variants, we ask that you please stay home and contact IAO staff immediately.
All attendees are required to agree to the IAO Liability Waiver when registering for the event.
