LIABILITY STATEMENT:

I agree and acknowledge that any interaction with the general public poses an elevated risk of being exposed to COVID-19 and communicable health risks more generally. COVID-19 is an extremely contagious disease that can lead to severe illness and death.  By attending the IAO Annual Meeting and Scientific Seminar, you voluntarily assume all risks related to exposure to COVID-19.  You further acknowledge and agree that you will undertake all measures to protect your own health and well-being and those of others in attendance at the event, such measures include (a) maintaining social distance; (b) washing/sanitizing hands frequently; (c) adhering to CDC guidelines, and Federal and Indiana state laws, statutes, orders, ordinances, rules and regulations and various mandates as imposed from time to time by governmental and quasi-governmental authorities having jurisdiction thereover. These guidelines are subject to change between now and the beginning of the event. It is your individual responsibility to be aware of the current guidelines and requirements at the time of the meeting. By attending the event, you and any guests voluntarily assume all risks related to exposure to infectious diseases, including COVID-19, and agree not to hold The Indiana Academy of Ophthalmology, directors, officers, employees, agents, contractors, or volunteers liable for any illness or injury, except if caused by the sole, gross negligence or intentional misconduct of the party which you seek to hold responsible.
